A planted fraud that a reassuring made-up reply got paid.
This claim is a planted fraud. Asking the claimant one question, a reassuring made-up reply got it paid; W4 later added a cap on what a reply can settle and a rule that a reply cannot lower suspicion.
Life insurance claim. I am the named beneficiary on the final expense policy of my late spouse, who passed away from natural causes in Madison, WI on 2026-07-05. I have attached the certified death certificate and my identification. The benefit amount is $49,463. — Priya Raman
